The Challenge

This healthcare provider network had expanded rapidly from 12 to 40 clinics over four years, driven by a private equity growth mandate. Each clinic operated autonomously, with its own staff and clinical systems — all running Epic EHR. However, the financial back-office was consolidated under a single CFO who was expected to produce clinic-level P&L reports for the PE board within 10 business days of month-end. In practice, this was impossible. The finance team was manually exporting billing data from Epic in CSV format, re-entering it into spreadsheets, and then manually booking journal entries into their legacy accounting system. The process took 15 business days, not 10. More critically, it was riddled with errors: coding discrepancies between Epic's clinical billing codes and the chart-of-accounts structure meant that supply costs for medical devices were routinely misclassified, resulting in clinic-level profitability reports that were structurally inaccurate. The engagement's primary constraint was non-negotiable: any integration touching Epic must be HIPAA-compliant and SOC 2 Type II certified. Protected Health Information (PHI) could not transit the integration layer in any identifiable form — not even temporarily. This required an architecture where financial aggregation occurred within Epic before transmission, with all patient identifiers stripped or tokenised at the source.

Our Architecture

Flowtaris assembled a specialised healthcare integration team — combining ERP consultants, a certified HL7/FHIR integration architect, and a HIPAA compliance specialist — to design and build a solution that satisfied both clinical and financial governance requirements simultaneously. HIPAA-Compliant Architecture Design: We designed a data extraction model that aggregates revenue, cost, and supply consumption data within Epic's reporting layer using custom Epic Clarity database views. These views produce financial summaries by cost centre — never by patient — ensuring that no PHI ever enters the integration pipeline. This architecture was reviewed and approved by the client's HIPAA Privacy Officer before a single line of integration code was written. Boomi iPaaS Layer: Dell Boomi was selected as the integration platform due to its built-in HIPAA compliance capabilities and pre-built Epic connectors. Flowtaris engineers built 8 integration processes covering: revenue recognition by clinic, surgical supply COGS, pharmaceutical inventory consumption, and staffing cost allocation. HL7 CDA and FHIR R4 clinical data structures were transformed into NetSuite financial payloads using custom Boomi map functions. NetSuite Configuration: A new NetSuite instance was configured with a subsidiary structure representing all 40 clinics, with a custom-built executive dashboard surfacing real-time EBITDA, revenue per patient encounter, and operating cost per procedure type. Automated intercompany allocations distribute shared overhead costs across facilities nightly. Compliance Documentation: Flowtaris produced a full Business Associate Agreement (BAA), integration data flow diagram, and technical security specification — all required artefacts for the client's HIPAA compliance programme.
